The Clinical Pharmacy Specialist (Preventive Medicine and Primary Care) at the Lexington VA Health Care System functions as a physician extender with a complete Scope of Practice.

You will conduct complete patient health appraisals, including health histories, drug histories, and physical examinations, and document findings in the medical record. You will evaluate drug therapy through direct patient care, assessing responses to therapy and communicating recommendations.

With approved Scope of Practice and under attending physician supervision, you will have limited authority to direct drug therapy, order and interpret laboratory tests, and place consults.

Qualification

To qualify for this GS-13 Clinical Pharmacy Specialist position, you must meet basic requirements including U.S. Citizenship, English proficiency, and graduation from an ACPE-accredited pharmacy program with a Pharm.D. Or BS Pharmacy. A full, current, unrestricted pharmacist license in a state, territory, or D.C. Is required.

  • U.S. citizenship required; noncitizens may be appointed only when qualified citizens cannot be recruited.
  • Proficiency in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d).
  • Graduate of an ACPE-accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.
  • Graduates of foreign pharmacy programs must provide FPGEC certification, including passing the FPGEE and TOEFL iBT.
  • Full, current, and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the U.S., or the District of Columbia.
  • Must maintain current registration if required for licensure.
  • Pharmacists with a history of license revocation, suspension, or restriction may be appointed only per VA Handbook 5005 provisions.
  • Unlicensed pharmacists must obtain licensure within 2 years of appointment or face removal.
  • Grandfathering provision applies to VHA pharmacists employed on the effective date of the qualification standard.
  • GS-13 level requires specialized experience and/or education as determined by grade-level standards.
Role Responsibilities

Conduct complete appraisals of patients' health status by taking health histories, drug histories, and performing physical examinations.

  • Document relevant findings in the patient's medical record.
  • Evaluate drug therapy through direct patient care involvement, including clinical assessment and objective findings.
  • Communicate and document findings and recommendations to appropriate individuals and in appropriate records.
  • With approved Scope of Practice and under attending physician supervision, assume limited authority to direct drug therapy.
  • Order, perform, review, and/or analyze appropriate laboratory tests and other diagnostic studies to monitor and support drug therapy.
  • Place consults as authorized within Scope of Practice.
  • Serve as a physician extender in Preventive Medicine and Primary Care settings.
  • Collaborate with the healthcare team to optimize patient outcomes.
  • Participate in quality improvement initiatives and medication use evaluations.
