Salary & Compensation
Salary: $133,409 - $193,397 per year.
Qualification
To qualify, you must grip a post-master's certificate or a master's or doctoral degree as a Nurse Practitioner from an accredited program. You also require an active, unrestricted APN license and board certification as a Family, Adult, Adult-Gerontology Principal Care, or Gerontology Nurse Practitioner.
- Education: A post-master's certificate or master's or doctoral degree as a Nurse Practitioner from a scheme accredited by ACEN or CCNE.
- Foreign education must be evaluated by an agency like ICD.
- License: Active, existing, complete, and unrestricted APN license to practice as an NP in a U.S. State, territory, or D.C.
- Board Certification: Full and present certification as FNP, ANP, AGPCNP, or GNP from ANCC or AANPCB.
- English Language Proficiency: Must be proficient in written and spoken English.
- U.S. Citizenship called for; non-citizens may be appointed only if no qualified citizens are on offer.
- Selective Offering Registration for males born after 12/31/1959.
- Subject to background/security investigation and pre-employment physical examination.
- Participation in the seasonal influenza vaccination plan is required.

Duties & Responsibilities
- Independently handle a panel of patients with acute and chronic health conditions.
- Grow and put in place individualized blueprints for optimal population health and disease management.
- Perform finalize histories and physical exams.
- Offer fitting documentation including history, physical exam findings, diagnosis, care scheme, and disposition.
- Ensure each patient receives a list of existing medications and an appointment lineup at close-out.
- Collaborate with Leading Care Group and interdisciplinary team participants.
- Show leadership and give direction to back workforce assigned to the Patient Aligned Care Crew (PACT).
- Prescribe medications and treatments within the scope of practice and clinical privileges.
- Deliver evidence-based care emphasizing health promotion and disease prevention.
- Participate in caliber improvement initiatives and outcome-focused evaluations.
