The Clinical Pharmacist at the Richard L. Roudebush VA Medical Center in Indianapolis, IN, offers wrap up outpatient pharmacy services.

Qualification
To qualify for this GS-12 Clinical Pharmacist role, you must be a U.S. Citizen with a full, existing, and unrestricted license to practice pharmacy in a state, territory, or the District of Columbia.
You must be a graduate of an ACPE-accredited pharmacy plan with a BS Pharmacy or Pharm.D. Degree.
- United States Citizenship needed; non-citizens may be appointed only when qualified citizens can’t be recruited per VA policy.
- Packed, present, and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (e.g., Puerto Rico), or the District of Columbia.
- Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.
- Graduates of foreign pharmacy programs must offer proof of Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, covering passing the FPGEE and TOEFL iBT.
- Proficiency in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d).
- Must display expertise of expert pharmacy practice, aptitude to communicate orally and in writing, knowledge of laws and regulations for controlled substances, and competence in monitoring drug therapies.
- Candidates pending completion of educational or licensure conditions may be referred but can’t be hired until all needs are met.
- Existing permanent VA employees may qualify under the Grandfathering Provision if they gather the criteria in VA Handbook 5005.
Duties & Responsibilities
- Give timely responses to drug information requests from patient care providers and nurses.
- Evaluate healthcare provider orders for appropriateness of therapy, incompatibilities, side effects, and drug interactions.
- Use patient care info systems such as EHMR to obtain critical patient information and document clinical problems.
- Counsel patients on drug-drug interactions, adverse effects, food-drug interactions, item availability, and pharmacy policies.
- Manage drug recalls and medication shortages by substituting alternate dosage strengths or offerings per facility policy.
- Enter supply orders proper for patient care per facility policy.
- Run medication orders including flagging prescriptions, managing view alerts, using the hold function, and generating unsigned discontinued orders.
- Extend refills until the next scheduled appointment under the provider's name per facility policy.
- Supply medication employ counseling at the time of patient encounter.
