Job Description

The State of Washington is hiring a Part-time Case Manager for Aging and Adult Services of Coastal Washington (AASCW) located in Long Beach, WA.

In this role, you’ll provide assessment, reassessment, and in-home care management services to frail and vulnerable adults who have been found eligible for Title XIX services by the Department of Social & Health Services.

You’ll collaborate with clients and decision-makers to build service blueprints that maximize independence and grade of life.. Coordinate and put in place schemes within funding source needs and agency benchmarks.. And function as portion of a multi-agency service delivery team to offer a broad spectrum of needed services..

Such as advocacy under the Federal Older Americans Act. This role is ideal for experienced social offering professionals who are committed to supporting vulnerable populations and securing they get the care they need.

Salary & Compensation

Salary: $33.87 - $44.44 per hour.

Qualification
