Registered Nurse - Patient Aligned Care Team (PACT) - Togus

To qualify, you must be a U.S. Citizen proficient in English, hold a current, full, active, and unrestricted RN license, and be a graduate of an accredited nursing program (ADN, diploma, BSN, or MSN). Grade determination (VN 00) is based on education and experience.

• U.S. Citizenship; non-citizens may be appointed only when qualified citizens cannot be recruited.
• English Language Proficiency: Must be proficient in basic written and spoken English (38 U.S.C. 7403(f)).
• Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by ACEN or CCNE at the time of program completion.
• Individuals attending a master's level bridge program who have completed coursework equivalent to a bachelor's in nursing may be appointed temporarily upon state licensure, then converted to permanent after bridge program completion.
• Graduates of foreign nursing schools must have a current, full, active, and unrestricted registration that meets the requirement for an associate degree/entry level nursing education. Credit for higher education requires a formal degree equivalency evaluation from a recognized service such as International Consultants of Delaware (ICD).
• Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory, Commonwealth (e.g., Puerto Rico), or the District of Columbia.
• Graduate Nurse Technician (GNT) Exception: Candidates who meet education requirements but lack licensure may be appointed as a GNT on a 120-day temporary appointment while pursuing licensure (extendable up to two years).
• Grade Determinations: Nurse I, Level I (ADN/diploma, no experience); Nurse I, Level II (ADN/diploma + 1 year experience, or BSN); Nurse I, Level III (ADN/diploma + 2 years experience, or BSN + 1 year, or MSN); Nurse II (BSN + 2 years, or MSN + 1 year, or doctoral degree); Nurse III (MSN + 2 years specialized experience, or doctoral degree + 1 year specialized experience).
• Experience must demonstrate competencies in five dimensions: Practice, Veteran/Patient Driven Care, Leadership, Professional Development, and Evidence-Based Practice/Research.
• All applicants tentatively selected are subject to urinalysis to screen for illegal drug use prior to appointment.

Provide direct veteran care and care coordination within Primary Care.

• Manage assigned patient panels, use registries, and ensure appropriate evaluation and access.
• Provide health promotion, health education, and coaching on wellness, disease prevention, and chronic care management.
• Formulate a plan of care based on nursing assessments.
• Delegate elements of care to appropriate individuals based on assessed needs, condition, and patient safety.
• Seek guidance on how to improve care provided to patients and families.
• Apply primary care principles and theory with critical thinking skills.
• Responsible and accountable for individual nursing practice and determining appropriate delegation of tasks.
• Individual growth and development in outpatient PACT nursing practice with increased levels of self-direction.
