To qualify for this GS-11 Social Worker role, you must be a U.S. Citizen and have a master's degree in social operate from a CSWE-accredited school. Run home assessments as portion of the submission or reassessment process for the Plan of complete Assistance for Family Caregivers.

• Participate in face-to-face or virtual contact with the caregiver and Veteran to assess function and demand for personal care solutions.
• Apply and keep up a figures monitoring system to measure program efficacy and support with collecting and analyzing nearby records reports.
• Advise and collaborate with interdisciplinary teams throughout the medical center on caregiver concerns.
• Create educational tools, build programs, and put in place training focused on particular caregiver needs and problems.
• Carry out Veteran, caregiver, and family clinical interventions to pinpoint caregiver pressure, psychiatric illness, emotional distress, and social needs as treatment matters.
• Offer referrals to VA medical and mental health treatment, including VA gains, community-based social offerings, and other non-VA entitlements.
• Serve as a consultant for managing involved procedures that caregivers, Veterans, and families encounter when interacting with public sector and community agencies.
