The Columbia VA Health Care System is hiring a Social Worker (Program Coordinator) to manage the Dom VA Medical Center Respite Services Plan at the Greenville community-based outpatient clinic (CBOC).

In this GS-12 role, you’ll also cater to as the Lead Social Worker in the Patient Aligned Care Crew (PACT) for the CBOCs.

You’ll screen Veterans for enrollment in the inpatient respite scheme and recommend non-institutional care choices such as CADHC or in-home respite when inpatient beds are unavailable.

Qualification

To qualify for this GS-12 Social Worker (Program Coordinator) role, you must meet fundamental requirements such as U.S. Citizenship, English proficiency, and a master's degree in social function from a CSWE-accredited scheme. Role Responsibilities

Serve as the Social Perform Coordinator for the Dom VA Medical Center Respite Solutions Program at the Columbia VAMC CBOC location.
