Role Responsibilities

Coordinate reentry services for incarcerated individuals transitioning back to the community.

  • Develop and implement case plans in collaboration with the CBI Unit team.
  • Connect individuals with community resources and support services.

  • Collaborate with other DOC staff and external partners to ensure continuity of care.
  • Participate in unit meetings and training as required.
  • Maintain accurate records and reports in accordance with DOC policies.
